TRIAGE-PST: A Proposed Framework to Empower Community Health Workers with AI-Enhanced Problem Solving Therapy Skills for Mental Health Workforce Support During Public Health Emergencies

In the wake of public health emergencies, such as natural disasters, epidemics, and conflicts, mental health needs often surge at the same time baseline resources may be disrupted , further exacerbating already-existing service gaps. The Technology-Responsive Integrated Approach for General Emergencies (TRIAGE) model aims to address such gaps in service capacity by leveraging a triadic relationship between clients, and a Community Health Workers (CHW) / AI agent partnership. This session summarizes an extension of this model to TRIAGE-PST, which focuses on upskilling CHWs in Problem Solving Therapy (PST) in order to provide effective mental health support during emergencies. The primary objectives of TRIAGE-PST are to: (1) Equip CHWs with PST skills to address common mental health service needs in emergency contexts; (2) Integrate PST into the overall TRIAGE framework, enhancing the model's responsiveness to mental health needs, improve mental health outcomes, and reduce service gaps resulting from public health emergencies. The TRIAGE-PST design includes a comprehensive training curriculum for CHWs, covering collaborative skills in AI-partnered PST techniques and their application in public health emergency scenarios. Outcomes evaluation measures include changes in mental health indicators among clients served by CHWs, AI-assessed CHW competency in delivering PST, and overall satisfaction with the TRIAGE-PST program. The TRIAGE-PST program represents a novel integration of AI-supported psychological therapy skills into the CHW workforce, enhancing their potential role in emergency mental health support. The use of PST, a brief and evidence-based intervention, aligns well with the time-sensitive and resource-constrained nature of the emergency response continuum. Moreover, the inclusion of an AI conversational agent and decision support tool in the TRIAGE framework facilitates continuous monitoring and adaptation of the intervention, ensuring its relevance and effectiveness. By leveraging the accessibility and flexibility of the CHW workforce, TRIAGE-PST has the potential to provide timely and effective mental health support, contributing to the overall resilience of communities in the face of public health crises.
