Navigating Continuity of Care Challenges Through a Strategic Network ApproachProblem Statement: Frequently, multiple (mental) health care providers are involved in care trajectories of individuals at risk of suicide. Achieving continuity of care, however, is challenging. Ambiguity about the roles and responsibilities of health care professionals and organizations, particularly regarding accessibility, transfer and feedback, jeopardizes well-coordinated (after)care for those with suicidal thoughts or individuals who have attempted suicide. A strategic approach for mental health care networks is imperative.
Objective: At a regional level, multiple stakeholders are involved in this process, ranging from gatekeepers to various (mental) health care providers. To optimize continuity of care at the network level, a targeted strategic approach has been developed. This approach includes five phases, from start-up (exploring the strengths and weaknesses in continuity of care) to consolidation (solidifying regional agreements). Facilitated by both the network coordinator and the regional suicide prevention worker, this process aims to effectively address the four critical moments in care: accessibility, collaboration, transfer moments, and care refusal.
Results: Within this suicide prevention care network a shared vision and a common language are established, including clear agreements on information transfer to the next caregiver and a comprehensive understanding of the services provided by each (mental) health organization. This results in a clearer care flow for individuals at risk of suicide within the regional network. Network participation offers an opportunity to familiarize oneself with other (mental) health care providers, gaining insights into their respective roles and contributions within the professional landscape.
Evaluation: Sustaining the suicide prevention care network requires continuous effort. The goal is for everyone to actively participate, emphasizing ownership from each involved network partner. This is a dynamic and ongoing process that addresses emerging needs, changes in (health) care services, and (inter)national crises/trends.
Conclusion: This strategic approach ensures the establishment of a solid care network that acts as a continuous learning system and self-evaluating system, with the ultimate goal of ensuring continuity of care for individuals at risk of suicide.
