PS23 - Innovations in Suicide Prevention Crisis Intervention Services: Recent Challenges, Research and Evaluations

Early Outcomes of Canada's 9-8-8: Suicide Crisis Helpline

Canada implemented 9-8-8: Suicide Crisis Helpline (www.988.ca), a 3-digit suicide prevention service in November 2023, available 24/7/365 in French and English, by phone and SMS text. The introduction of 9-8-8 aligns with the growing international trend of adopting a national 3-digit access point and crisis service model. Crisis lines are an evidence-based intervention in a continuum of suicide prevention-intervention approaches and are utilized by those at elevated risk for suicide.1 The 9-8-8 intervention is offered by a network of 40 partners from across Canada, co-ordinated through a central hub. Foundational values for 9-8-8 are non-judgement, compassion, inclusion, cultural safety, and a community-based approach to suicide prevention. Implementation, quality improvement, and evaluation are priorities for the 9-8-8 service, and guided this initial rapid implementation. Both recent academic scholarship and operational approaches have highlighted the importance of using rigorous implementation science to ensure that suicide prevention initiatives meet their intended aims.2 Using the Pragmatic, Robust, Implementation, and Sustainability Model (PRISM)3 we will present on associated implementation, service, and service user outcomes over the first six months of the service, with a focus on understanding both implementation and quality of the service, including timeliness, safety, effectiveness, efficiency, and equity. The 9-8-8 service has received approximately 1,500 calls and 500 texts per day since launch, with a projection of approximately 550,000 interactions over the first 12 months; this is projected to increase to 1Million interactions per year after that. We will discuss our approach to implementing 9-8-8, lessons learned, and current strategies for quality improvement, with a focus on equity. We will include a discussion of our 50+ community consultations with diverse, equity-seeking groups across the country, including health equity impact assessments (HEIAs) that will contribute to making 9-8-8 a trusted and safe environment for groups that may be at an elevated risk of suicide.
